UPSEE 2014 - Application Form
Candidates are required to fill the UPSEE 2014 Application Form to apply for Uttar Pradesh State Entrance Examination 2014. Candidates can fill the form in two modes, online and offline.
Offline Mode
Application form can be purchased from head post offices of the state from around March 2014 onwards. The cost of application form is Rs.1000 for general and OBC candidates and Rs.500 for female, SC, ST candidates. Candidates are required to fill the form correctly and send it to the following address via speed post or registered post before the deadline. Any form that is received after the last date will not be accepted in any case. Address to send the application form is:

Online Mode
Alternatively, candidates can fill the application form online as well on the official website of UPSEE. Online application form will be issued around mid week of February 2014. Candidates who find it difficult to fill the form due to lack of computer knowledge or resources can visit the nearby facilitation center for assistance.
List of such centers will be uploaded on the official website itself. On opening the website, candidates have to click the link titled as “apply online”. On clicking the link, they would be shown various instructions to be followed while filling the application form. Afterwards, fill the form using correct and complete information. Verify all the fields and submit the application form.
A unique system generated application number will be issued to candidate after the form is submitted. It is required to preserve the application number till the admission process is complete since it’d be asked at various stages. The last and final step is to pay the application fee which can be paid via credit/debit card, bank challan or internet banking.
Bank Challan
Candidates can download the bank challan from the official website to remit the fee in bank. The challan will be in triplicate form where one copy is for bank, one for candidate and one to be sent with the application form. Challan will mention the fee details and candidate’s application number. In the bank, candidate have to deposit the challan along with application fee. The challan would then be stamped by the bank clerk. Also, candidate will be assigned a unique transaction number. Candidate can thus generate the confirmation page from official website by entering application number and transaction number.
Internet Banking OR Credit/Debit Card
Pay the application fee via online details entering of credit or debit card or via internet banking.
Afterwards, candidates must generate the printout of the confirmation page, put signatures and photos wherever necessary. Send the form to the following address thru registered post or speed post or in person:
